Please fill out the form located on the front page of the Nelson website in the top right corner.  Please remember to include your child’s full name, the reason for the absence, and list any symptoms if the absence is illness related.  Phone calls are not accepted to excuse absences.  Parents are allowed 8 parent note excuses per school year (4 per semester).  For medical or dental excuse notes, you can email an image of the note, turn it in at the front office, or they can be faxed to 463-633-4110.
