If you would like for your child to have Tylenol, Motrin, Benadryl, Cough drops, Tums, or Throat strips in the clinic please fill out this form. Your child can have 10 doses of these approved over the counter medications. A Doctor note must be obtained after the 10 doses. Any other over the counter medication MUST have a doctor’s signature on this form.
