At Maus, we want to keep your child as safe and healthy as possible. Please fill out the appropriate forms based on your child’s medical conditions. Once the forms are completed, they can be turned into or faxed to the Maus clinic. I look forward to caring for your child!

If you would like for your child to have Tylenol, Motrin, Benadryl, or Tums in the clinic please fill out this form. Your child can have 10 doses of these approved over the counter medications. A Doctor note must be obtained after the 10 doses. Any other over the counter medication MUST have a doctor’s signature on this form.
