Athletics Eligibility Checklist

In order for your child to be eligible to compete in any athletic sport with the school they must complete all information required before the start of that sport.  Once this information is complete the student may participate or tryout for any sport we offer.

• Physical and Medical History Form

The form can be found by clicking here.  Please have your physical scheduled for after May 1st of 2020.  When completed please keep a record for yourself and turn in a copy to the coaches at PHMS.  This Physical will be good for one school year.

• Impact Test

The impact test can be found by visiting www.impacttestonline.com/testing (please use chrome).  A mouse is needed to correctly take the baseline concussion test.  When completed please keep a record for yourself and turn in a copy to the coaches at PHMS.
